What is your fee?

My fee is $170 for a 53 minute session. I accept cash, credit, or check. Payment is expected at the time of service. Please see my disclosure form for further information.
 

Do you take insurance?

I am a contracted provider for the following insurance plans: 

  • First Choice Health

  • Kaiser PPO plans

  • LifeWise

  • Premera (and most out of State Blue Cross Blue Shield plans)

I think you would be a good fit, but I don't see my insurance plan listed.

I encourage you to call the number on the back of your insurance card to find out what your out-of-network benefits are. Using your out-of-network benefits means that you are responsible for the fee at the time of service, but I can provide you with the appropriate documentation that you can submit to your insurance company for possible reimbursement. This form can also help you determine your out-of-network benefits.

 

WHERE IS YOUR OFFICE? IS IT ACCESSIBLE?

My office is in Mountlake Terrace, about five blocks away from I-5 and the Mountlake Terrace Transit Center. Click here for a map. The office has a designated parking lot and is ADA accessible.
 

What do I need to do prior to my first appointment?

Once we have scheduled an appointment, you will receive an email with a link to my online portal. It will direct you to create a login and fill out my intake paperwork. Usually, it takes around 30-45 minutes to fill out this information and sign the forms.
